Book "Food Chemicals Codex 5th Edition"

Upaya kesehatan adalah setiap kegiatan untuk memelihara dan meningkatkan kesehatan, bertujuan untuk mewujudkan derajat kesehatan yang optimal bagi masyarakat. Upaya kesehatan diselenggarakan dengan pendekatan pemeliharaan, peningkatan kesehatan (promotif), pencegahan penyakit (preventif), penyembuhan penyakit (kuratif), dan pemulihan kesehatan (rehabilitatif), yang dilaksanakan secara menyeluruh, terpadu, dan berkesinambungan. Konsep kesatuan upaya kesehatan ini menjadi pedoman dan pegangan bagi semua fasilitas kesehatan di Indonesia termasuk rumah sakit. Rumah sakit yang merupakan salah satu dari sarana kesehatan, merupakan rujukan pelayanan kesehatan dengan fungsi utama menyelenggarakan upaya kesehatan yang bersifat penyembuhan dan pemulihan bagi pasien.

Pelayanan farmasi rumah sakit merupakan salah satu kegiatan di rumah sakit yang menunjang pelayanan kesehatan yang bermutu. Hal tersebut diperjelas dalam Keputusan Menteri Kesehatan Nomor 1333/Menkes/SK/XII/1999 tentang Standar Pelayanan Rumah Sakit, yang menyebutkan bahwa pelayanan farmasi rumah sakit adalah bagian yang tidak terpisahkan dari sistem pelayanan kesehatan rumah sakit yang berorientasi kepada pelayanan pasien, penyediaan obat yang bermutu, termasuk pelayanan farmasi klinik, yang terjangkau bagi semua lapisan masyarakat.

Tuntutan pasien dan masyarakat akan mutu pelayanan farmasi, mengharuskan adanya perubahan pelayanan dari paradigma lama (drug oriented) ke paradigma baru (patient oriented) dengan filosofi Pharmaceutical Care (pelayanan kefarmasian). Praktek pelayanan kefarmasian merupakan kegiatan yang terpadu dengan tujuan untuk mengidentifikasi, mencegah dan menyelesaikan masalah obat dan masalah yang berhubungan dengan kesehatan.

Buku Standar Pelayanan Kefarmasian di Rumah Sakit ini sangat penting dalam rangka penerapan paradigma pelayanan kefarmasian, yaitu konsep Pharmaceutical Care yang sesuai dengan tuntutan masyarakat yang mengharuskan adanya perubahan pelayanan dari drug oriented ke patient orinted, namun dalam pelaksanaan pedoman ini juga sangat perlu didukung oleh komitmen dan kemauan tenaga farmasi dalam menjalankannya.

Berbagai upaya telah dilakukan untuk meningkatkan mutu pelayanan kefarmasian, namun kenyataannya dari monitoring yang pernah dilakukan menunjukkan bahwa pelayanan kefarmasian di Puskesmas belum diterapkan secara optimal. Beberapa faktor yang menjadi penyebabnya antara lain karena belum adanya standar, kemampuan tenaga farmasi serta pihak-pihak yang terkait tentang pelayanan kefarmasian maupun kebijakan manajemen dari Puskesmas itu sendiri serta pelaksana pelayanan kefarmasian di Puskesmas belum semuanya apoteker atau asisten apoteker sehingga memberikan dampak terhadap mutu pelayanan kesehatan. Oleh sebab itu tenaga farmasi dituntut untuk meningkatkan pengetahuan, keterampilan dan perilaku agar dapat langsung berinteraksi dengan pasien.

Untuk mewujudkan visi tersebut ditetapkan misi pembangunan kesehatan yang salah satunya adalah menjamin tersedianya pelayanan kesehatan yang bermutu. Untuk itu diperlukan perubahan dalam sistem pelayanan kesehatan termasuk di dalamnya pelayanan kefarmasian. 

Pada saat ini orientasi paradigma pelayanan kefarmasian telah bergeser dari pelayanan obat (drug oriented) menjadi pelayanan pasien (patient oriented) dengan mengacu kepada Pharmaceutical Care. Kegiatan pelayanan yang tadinya hanya berfokus pada pengelolaan obat sebagai komoditi berubah menjadi pelayanan yang komprehensif dengan tujuan untuk meningkatkan kualitas hidup pasien. 

Sebagai konsekuensi perubahan orientasi tersebut maka apoteker dituntut untuk selalu meningkatkan pengetahuan dan keterampilannya agar mampu berkomunikasi dengan tenaga kesehatan lain secara aktif, berinteraksi langsung dengan pasien di samping menerapkan keilmuannya di bidang farmasi. Berdasarkan hal tersebut, Departemen Kesehatan bekerja sama dengan Ikatan Sarjana Farmasi Indonesia (ISFI) telah menyusun Standar Pelayanan Kefarmasian di Apotek. Buku Standar Pelayanan Kefarmasian di Apotek ini merupakan suatu pedoman praktik Apoteker di Apotek untuk meningkatkan mutu pelayanan.

Book "Daftar Obat Esensial Nasional (DOEN) 2008"

Daftar Obat Esensial Nasional (DOEN) merupakan daftar obat terpilih yang paling dibutuhkan dan diupayakan tersedia di unit pelayanan kesehatan sesuai dengan fungsi dan tingkatannya. DOEN merupakan standar nasional minimal untuk pelayanan kesehatan. DOEN direvisi dan disempurnakan secara berkala.

Konsep Obat Esensial di Indonesia mulai di perkenalkan dengan di keluarkannya DOEN yang pertama tahun 1980, dan dengan terbitnya kebijakan Obat Nasional pada Tahun 1983. DOEN direvisi secara berkala setiap 3-4 tahun. DOEN yang terbit sekarang ini merupakan revisi tahun 2008. Menteri Kesehatan Republik Indonesia telah mengeluarkan Surat Keputusan Menteri Kesehatan Republik Indonesia Nomor 791/Menkes/SK/VIII/2008 tentang Daftar Obat Esensial Nasional 2008 ini pada tanggal 21 Agustus 2008 yang lalu.

Penerapan DOEN dimaksudkan untuk meningkatkan ketepatan, keamanan, kerasionalan penggunaan dan pengelolaan obat yang sekaligus meningkatkan daya guna dan hasil guna biaya yang tersedia sebagai salah satu langkah untuk memperluas, memeratakan dan meningkatkan mutu pelayanan kesehatan kepada masyarakat. DOEN merupakan dasar untuk pedoman perencanaan dan pengadaan obat di berbagai fasilitas pelayanan kesehatan pemerintah seperti puskesmas, rumah sakit , dan fasilitas pelayanan kesehatan lainnya baik di tingkat pusat maupun daerah (kabupaten/kota).

Revisi DOEN dilaksanakan secara periodik setiap 3 tahun untuk menyesuaikan dengan kemajuan ilmu pengetahuan juga untuk kepraktisan dalam penggunaannya. DOEN 2008 merupakan revisi yang ke-9 dan merupakan revisi Reformasi untuk menindaklanjuti survey WHO mengenai Measuring Transparancy to Improve Good Governance in Public Sector. Pada revisi 2008 pengkajian bukan hanya membahas usulan yang disampaikan oleh pengusul melainkan juga mengkaji ulang seluruh DOEN 2005. Dalam buku DOEN 2008 ini, anda akan menemukan daftar obat yang disajikan dalam bentuk Kelas Terapi (nama generik), Formulasi (bentuk sediaan, kekuatan, dan kemasan), dan Restriksi dari obat tertentu. Tidak hanya itu, DOEN 2008 juga memuat Daftar Obat Terbatas untuk Puskesmas dan Daftar Obat dalam DOEN 2005 yang Mengalami Perubahan. Berikut adalah perubahan-perubahan yang terdapat dalam DOEN 2008 :

  • 48 obat ditambahkan ke dalam DOEN 2008
  • 21 obat mengalami perubahan formulasi

Buku ini sangat sakti, mengapa? Karena berisi soal plus pembahasan lengkap 12 mata kuliah. Dikatakan sakti karena sangat ampuh buat belajar sistem SKS, dan bakal menjadi bacaan wajib ketika ujian. Dan juga perlu diketahui, buku ini adalah “harta” yang tak ternilai harganya, ditulis dengan keikhlasan hati, kesuciam jiwa, ketekunan, kesungguhan, kerja keras, perjuangan, demi mencapai masa depan nan gemilang. Opo tho yo...

Thanx buat teman-teman Profesi 2008 Kelas C (Farmasi Sains dan Industri) yang saya banggakan. Tuh, bener khan. Saya bilang juga apa, kita pasti bisa. Bisa apa? Bisa bekerja sama dalam keluarga, saling bahu dan membahau sehingga buku ini ada di tangan teman-teman semua dan siap baca, enak dibawa menjelang ujian dan bisa dirasakan manfaatnya.

Emang, dalam membuat buku ini susah, nyari-nyari literatur di perpus, bongkar-bongkar file di CD, obrak-abrik catetan kuliah, bahkan saya dapet laporan katanya tim Farmakoterapi sampe lembur jam 2 pagi, bahkan tim MFK dibela-belain sampe jam 3 pagi. Wow, kurang apa coba? Demi kita, demi calon apoteker baru, untuk merintis kehidupan yang lebih baik dan Indonesia yang maju.

Oya, mungkin dalam menjawab soal-soal banyak kesalahan kami mohon maaf, semoga rekan-rekan calon sejawat bisa memaklumi dan memberi masukan untuk penerbitan yang lebih baik di tahun depan.

Book "Pharmaceutical Care Practice: The Clinician's Guide"

The clue to the objective of this important new book is in its title. The first edition, published in 1998, was just called "Pharmaceutical care practice." This time the phrase "the clinician's guide" now appears on the cover along with the main title. The first book was somewhat abstract, concerned more with the theory and politics of progressing a new way for pharmacists to practise (new for pharmacists, that is, but not other health professionals). The second edition is aimed at practitioners and is intended for their direct use. This is an intentional change on the part of the authors and I am sure it is a correct one. The need for improvement in the way that patients use their medicines is well recognised at government level. What is now needed is practical means of achieving that end in a concerted way. This book addresses that need.

But while the publication may have changed, the underlying philosophy has not. This is that a practitioner (preferably a pharmacist) should assume responsibility for a patient's drug-related needs and be held accountable for this commitment. This is the way that pharmaceutical care practice is defined. And this is what the authors, who are based at the Peters Institute of Pharmaceutical Care in the University of Minnesota, have spent their professional lives seeking to promote.

Cipolle, Strand and Morley see pharmaceutical care as a generalist practice with the pharmaceutical care practitioner assessing "all of a patient's medications, medical conditions and outcome parameters" and identifying, resolving and preventing "drug therapy problems."

The book describes how pharmacists should go about practising in this way. It sets out the role of the practitioner and that of the patient. It explains how to carry out an assessment of the patient's drug-related needs and categorises drug therapy problems and says how they can be identified. It gives details on the drawing up of a care plan with the patient and how this should be followed up.

Many pharmacists in England and Wales will be adopting the principles involved in pharmaceutical care practice as they carry out "medicines use reviews" and "medication reviews" under the new contract. Pharmacists in Scotland will be doing this increasingly, too, as the pharmaceutical care pilots become more widespread. All would be well advised to prepare themselves by acquiring books such as this.

Pharmacists will have to change their mind set. They are going to have to see the dispensing of a prescription as the start of a process. This will not be a voluntary thing. Patients expectations will increase through medicines use review and similar developments. The revolution has begun. It may be a little slow at first, but it will gain momentum. Books such as this are an important catalyst of change.

Book "Drug Information: A Guide for Pharmacists"

Over the last ten years, there has been an increasing realization of the importance of information. Much of this can be related to the increased availability of Internet information sources throughout society, along with the ease by which material can be located and used. The impact of the Internet can also been seen in this book. The first edition contained only two pages of information about the Internet, which reflected the small amount of medical information available and the little impact that it had on the profession at that time. In this new edition, it seems as if hardly a page can be found without some reference to Internet material. This increased emphasis on information has had an effect on both the health care professional, who uses the material, and the patient, who may look up material directly and even bring it in to talk about with a pharmacist or physician. The ability to obtain, manage, and use information has become an important core skill for the professional.


Unfortunately, pharmacists in practice may find it difficult to learn how to manage information, due to a lack of good, comprehensive resources to teach them proven methods for improving their skills. Students also need a source to supplement the classroom and clerkship training they receive. It is to serve those populations that this book was originally written. In this third edition, the goal of this book continues to be to educate both students and practitioners on how to efficiently research, interpret, collate, and disseminate information in the most usable form. While there is no one right method to do these things, proven methods are presented and demonstrated. Also, seldom-addressed issues are covered, such as the legal and ethical considerations of providing information.

The book begins by introducing the concept of drug information, including its history, and providing information on various places drug information specialists may be employed. The book continues on by describing the various steps for obtaining, evaluating, and providing information. As with the first edition, the “Modified Systematic Approach” to answering a question is presented. “Formulating Effective Responses” further expands on this topic by addressing problems that pharmacists experience when answering questions and providing techniques for overcoming these issues in order to reach appropriate conclusions. This section of the book is designed to teach pharmacists and students useful methods for determining what information is actually needed and how to adequately respond to requests.

Subsequent chapters allow the reader to further expand their skills in these areas. Once the pharmacist determines what information is needed using the skills outlined in the initial chapters, resources must be consulted to formulate a response. As always, a chapter discussing various resources that may be consulted for specific types of information has been provided, which has expanded coverage of electronic resources, particularly those for personal digital assistants (PDAs). New material on how to find information regarding veterinary medicine and complementary/alternative medicine has been added. A chapter on electronic information management is included, although there has been an effort to include this type of information throughout the book.

Even when information is found, pharmacists must evaluate the literature for quality and usefulness. The earlier editions of this book provided information on how to evaluate the medical literature. Those evaluation techniques are again in this edition with additional information being provided.

Two specific types of literature have been identified for even greater examination— pharmacoeconomics and evidence-based clinical practice guidelines. Information is presented on how to both perform such functions and evaluate work prepared by others. Evaluation of information resources often requires knowledge about statistical tests. The “Clinical Application of Statistical Analysis” chapter is an expansion of information provided in previous editions. The reader of this chapter will discover how to evaluate the appropriateness of statistical tests used in clinical studies.

Pharmacists may be asked to provide information in written form. The next chapter describes how this may be done. Additionally, sections describing how to prepare materials for formal presentations (platform and poster) and develop Websites are also provided.

The legal and ethical aspects of providing information always must be considered. The chapters on these topics have been updated and improved to be even more useful tham those in previous editions. In particular, additional information has been provided on new privacy regulations that have been instituted because of the Health Insurance Portability and Accountability Act of 1996 (HIPAA).

The remaining chapters deal with specialized functions that have often been the responsibility of drug information specialists but may be addressed by other pharmacists. These chapters will build upon the first part of the book. Much of the information in these chapters was covered in the Pharmacy and Therapeutics (P&T) chapter of the first edition; however, that chapter is now mostly limited to formulary management and some minor P&T functions. The formulary material also has increased information regarding third-party payer (e.g., insurance companies) formularies. New and expanded information is provided on quality assurance, adverse drug reactions and medication errors. Also, the information on how to prepare a drug evaluation monograph has been moved to a new chapter, with additional information on standards that have been prepared by the Academy of Managed Care Pharmacy (AMCP) and various governments. Finally, the chapter on Investigation Drugs has been updated to take into account new information and procedures.

With the veritable Niagara Falls of drug or pharmacy information available, much of which is complex, pharmacists have an increasing need for information management skills. This book will assist any pharmacist or student in the improvement of his or her skills in this area and allow individuals to evolve into new roles for the advancement of both the profession and care of patients.
